All forms must be left with the Front Desk, even if you have a scheduled appointment.
We do not discriminate or turn anyone down due to the inability to pay. We do provide services on a sliding scale basis. We also provide services to eligible uninsured people.

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a missed appointment fee for $100 cancellation policy.
